چکیده

Long-term survival of patients with chronic renal failure treated with regular haemodialysis has recently been recognized to be associated with peculiar complications which involve the osteoarticular system [1]. Such complications are related to a new form of systemic amyloidosis which is due to /?2-microglobulin (/J2-M), a protein which is catabolized by normal kidneys and accumulates when renal failure occurs [2]. This protein has a beta pleated sheet structure and therefore is able to form amyloid filaments. More commonly, /?2-M amyloid deposits are found in the osteoarticular system, where synovial structures are present.

Influence of heparin molecular size on the induction of C-terminal unfolding in β2-microglobulin

Dialysis-related amyloidosis (DRA) is characterized by accumulation of amyloid β2-microglobulin (β2m) in the interstitial matrix. Matrix substances such as heparin have reportedly been strongly implicated in the pathogenesis of dialysis-related amyloidosis. In clinical setting of hemodialysis, two types of heparin, i.e., high and low molecular heparin (H.M.H. and L.M.H.) have been routinely use...

Dramatic beneficial effect of interleukin-1 inhibitor treatment in patients with familial Mediterranean fever complicated with amyloidosis and renal failure.

BACKGROUND Familial Mediterranean fever (FMF) is an autosomal recessive autoinflammatory disorder, for which systemic AA amyloidosis is the major complication revealed most of the time by renal abnormalities. Current treatment is daily colchicine that prevents both recurrent inflammatory attacks and amyloidosis deposition in most patients. However, some patients still develop amyloidosis and re...

A Rare Presentation of Multiple Myeloma: A Case Report of Hepatic Amyloidosis

The clinically reported case of liver involvement with multiple myeloma (MM) is rare. Amyloidosis, defined as a tissue deposition of clonal light-chain fibrils, has been reported in 10-15% of the MM patients. We described a rare MM patient with the primal presentation of fulminant hepatic failure and biliary system involvement due to amyloidosis. Our patient had the primal symptoms of hyperbili...

Presentation and outcome of patients with systemic amyloidosis undergoing dialysis.

BACKGROUND AND OBJECTIVES Light chain (AL) and secondary (AA) amyloidosis usually present as a systemic disease frequently involving the kidney and leading to ESRD. Data regarding patients with AA or AL amyloidosis undergoing dialysis remain scarce. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS We retrospectively studied patients with AA or AL amyloidosis who started dialysis in five French c...

Rapidly progressive glomerulonephritis complicating primary AL amyloidosis and multiple myeloma.

Crescentic glomerulonephritis is a rare complication of AA amyloidosis. There are no clinical case reports of this complicating AL amyloidosis. A 67-year-old man developed rapidly progressive glomerulonephritis (RPGN) on a background of primary AL amyloidosis and IgGkappa multiple myeloma. Investigations for causes of glomerulonephritis were negative, and a renal biopsy confirmed crescentic glo...
